COLORADO SPRINGS, COLO. -- Memorial Hospital officials announced Tuesday they are missing health information for about 6,400 patients.

According to officials lab reports for work done between May and August are gone, but they don't think the information has been accessed by anyone.

The reports list patient names, Memorial internal account numbers, dates and the type of lab work that was done. Lab results, credit card information, demographic information and Social Security numbers were not part of the reports.

Investigators said they don't know how the information was lost.

“Patients entrust us with their private information, and we take the responsibility to protect patient information very seriously. We will use this as an opportunity to look for additional ways to better secure patient records. We deeply regret this error,’’ Mike Scialdone, Chief Executive Officer of Memorial Hospital, said in a statement.

Patients who have concerns related to this incident are asked to call (866) 283-9930.
